Highly Customizable Bone Fracture Fixation through the Marriage of Composites and Screws

چکیده

Open reduction internal fixation (ORIF) metal plates provide exceptional support for unstable bone fractures; however, they often result in debilitating soft-tissue adhesions and their rigid shape cannot be easily customized by surgeons. In this work, a surgically feasible ORIF methodology, called AdhFix, is developed combining screws with polymer/hydroxyapatite composites, which are applied shaped situ before being rapidly cured on demand via high-energy visible-light-induced thiol–ene coupling chemistry. The method porcine metacarpals transverse multifragmented fractures, resulting strong stable fixations bending rigidity of 0.28 (0.03) N m2 maximum load break 220 (15) N. Evaluations human cadaver hands proximal phalanx fractures show that AdhFix withstands the forces from finger flexing exercises, while short- long-term vivo rat femur fracture models successfully supports healing without degradation, adverse effects, or adhesions. This procedure represents radical new approach to fixation, grants surgeons unparalleled customizability does not

Optimizing the biomechanical compatibility of orthopedic screws for bone fracture fixation.

Progressive loosening of bone fixation screws is a well-documented phenomenon, induced by stress shielding and subsequent adaptive bone remodeling which results in bone loss around the screw. A set of two-dimensional computational (finite element) models was developed in order to test the effect of various engineering designs of fixation screws on the predicted screw-bone stress transfer, and c...

Magnesium-Based Absorbable Metal Screws for Intra-Articular Fracture Fixation

MAGNEZIX® (Syntellix AG, Hanover, Germany) is a biodegradable magnesium-based alloy (MgYREZr) which is currently used to manufacture bioabsorbable compression screws. To date, there are very few studies reporting on a limited number of elective foot surgeries using this innovative implant. This case report describes the application of this screw for osteochondral fracture fixation at the humera...

Fracture fixation with two locking screws versus three non-locking screws

OBJECTIVES We aimed to further evaluate the biomechanical characteristics of two locking screws versus three standard bicortical screws in synthetic models of normal and osteoporotic bone. METHODS Synthetic tubular bone models representing normal bone density and osteoporotic bone density were used. Artificial fracture gaps of 1 cm were created in each specimen before fixation with one of two...

Short Segment Fixation Versus Short Segment Fixation With Pedicle Screws at the Fracture Level for Thoracolumbar Burst Fracture

Objective: The most prevailing surgical procedure in the treatment of thoracolumbar burst fractures, Short Segment Fixation (SSF), is often followed by loss of correction or hardware failure which may be signifi cant enough to require another surgical intervention. In order to take advantage of its benefi ts but to avoid or diminish the risk and impact of associated drawbacks, some other altern...
